Added Manual Registration Tool with updated Median Filter tool

Recorrected a windows compilation issue (was in version 1.2 but had been removed by dsarrut in version 1.3)

Corrected tool mechanism for windows: static data members seem to be initialized only
at first instantiation and not as startup (as is the case with linux gcc) => dummy global.
Reimplemented singleton with private constructor and pointer member.

The new macro put the *_ggo* files in the binary directory so it's useless to ignore them.

Modified ggo mechanism: now uses a macro (WRAP_GGO) on the qt WRAP_UI model. The old mechanism is still in place but the new way should be used, e.g.:
